FAQ’s

  1. Q: What are the dates of the Sweepstakes?
    A: The Sweepstakes begins on or about June 1, 2009 and is scheduled to end on December 31, 2009, 11:59:59pm [CST]. A random drawing will be held for each Sweepstakes during the Promotion Period.
  2. Q: Who can enter in the Sweepstakes?
    A: The Sweepstakes is open to legal residents of the (50) United States and District of Columbia who are fifteen (15) years of age or older at time of entry.

Persons in any of the following categories are NOT eligible to participate or win prizes in the Sweepstakes: (a) persons who on or after October 1, 2006, were or are franchisees, directors, officers, employees or agents of McDonald's, prize suppliers, or their respective parent, subsidiary, or affiliated companies, franchisees, or service agencies, or independent contractors of any of the above organizations; (b) individuals engaged in the development, production or distribution of materials for the Sweepstakes; and (c) persons who are immediate family members (defined as spouse or biological or step- mother, father, sister, brother, daughter, or son) of any person in any of the preceding categories, regardless of where they live, or who reside in the same household, whether related or not, as any person in any of the preceding categories.

  1. Q: How do I enter?
    A:
    Online entry: Consumers who make a purchase at participating McDonald’s restaurants will receive a specially marked cash register receipt with sweepstakes rules on the back (while supplies last). To enter the sweepstakes, you will log-on to the Website www.mcdonalds-survey.com in the United States and you will be asked if you are 15 years or older to confirm eligibility. Users age 15 and older will be asked to provide receipt information for restaurant identification purposes and their first and last name, address, city, state/Province, zip/postal code, phone number and e-mail address for sweepstakes entry purposes. You will then be asked to answer survey questions relating to your McDonald’s visit.

Toll-Free Phone Number Entry Method: Consumers will also have the option to call the toll-free phone number if listed on the receipt and will be required to answer questions regarding your date of birth to confirm eligibility, receipt information for restaurant identification purposes and provide their first and last name, address, city, state/Province, zip/postal code and phone number for sweepstakes entry. You will then be asked to answer survey questions relating to your McDonald’s visit.

Mail-In entry: To enter without a McDonald's purchase, mail a 3" x 5" piece of paper with your name, address, city, state, and zip code and date-of-birth, legibly printed in blue ink to: Redemption Center, McDonald's ”Customer Satisfaction Survey” Sweepstakes Entry, P.O. Box 5000, Oak Brook, IL 60522-5000.

All mail-in entries must be postmarked no later than December 31, 2009 and received by the Redemption Center no later than January 8, 2010. The date on which a mail-in entry is received by the Redemption Center will determine the Entry Period for which the mail-in entry is eligible.

Mail-in entries must be mailed separately in separate outer envelopes or they will not be accepted. Mechanically reproduced or business reply envelope entries will not be honored.

  1. Q: How often can I enter?
    A:
    Each participant, phone number or email address may only be entered once per Entry Period by either going online at www.mcdonalds-survey.com for the United States, calling the toll-free phone number or by mail-in entry during the sweepstakes promotion. Limit one (1) entry per person or e-mail address or daytime phone number per Entry Period, regardless of entry method, i.e. online, toll-free IVR phone call or mail-in entry.

  1. Q: When will the drawings take place?
    A: Throughout the Promotion Period, there will be a series of thirty one (31) weekly and seven (7) monthly sweepstakes (collectively, the "Sweepstakes").   The Sweepstakes is scheduled to begin at 12:00:01 a.m. Central Time (“CT”) on or about June 1, 2009 and is scheduled to end at 11:59:59 p.m. CT on or about December 31, 2009 (the "Promotion Period"). Throughout the Promotion Period, there will be a series of random drawings to award prizes, for each of (31) weekly Sweepstakes and one for each of (7) monthly Sweepstakes. The “Entry Period” for each weekly Sweepstakes will begin on each Sunday during the Promotion Period, starting at 12:00:01 a.m. CT, and continuing until 11:59:59 p.m. CT the following Saturday, with the exception of the first week which will begin on Monday, June 1, 2009 and the last week which will begin on Sunday, December 27, 2009 at 12:00:01 a.m. CT and end on Thursday, December 31 2009 at 11:59:59 p.m. CT. The Entry Period for each Monthly Sweepstakes will begin at 12:00:01 a.m. CT on the first day of each month and end at 11:59:59 p.m. CT on the last day of each month. Entry Periods are subject to change, and any changes will be announced on the Website www.mcdonalds-survey.com in the United States and District of Columbia. Weekly entries will not be carried over to subsequent weekly periods and monthly entries will not be carried over to subsequent monthly periods.
  2. Q: How long will it take to verify the Official Winners?
    A:
    The potential First and Second Prize winners will be notified by phone, email and/or mail and will be required to complete an affidavit of eligibility and, where legal, a publicity release and a W-9 tax form, as set forth in the Official Rules of this Sweepstakes. Sponsor shall have no liability for any potential winner notification that is not received by any potential winner for any reason. If, despite reasonable efforts, any potential Prize winners do not respond within fourteen (14) days of the notification attempt, or if the prize is declined or a notification is returned as unclaimed or undeliverable to any potential winner, such potential winner will forfeit his or her prize and an alternate winner may be selected. If any potential winner is found to be ineligible, or if he or she has not complied with these Official Rules or declines a prize for any reason prior to award, such potential winner will be disqualified and an alternate winner may be selected.
  3. Q: What are the prizes?

    One (1) First Prize winner per monthly Entry Period wins $1,000 (awarded as a check in US). Ten (10) Second prize winners per weekly Entry Period will each receive a $25 McDonald’s Arch Card

If the First or Second Prize winner is a minor in his or her state of residence (a "Minor"), a parent or legal guardian will accept the prize on their behalf or will be asked to ratify and sign Prize Claim Documents.

  1. Q: What are my chances of winning?
    A:
    Odds depend on the number of eligible entries received for each drawing period.
  2. Q: When do I pay tax if I win a prize?
    A:
    You will only receive a 1099 tax form for winning the Grand Prize and you would receive the form for the year that you take possession of the prize. If you are under the age of 18, your parent or legal guardian will be responsible for paying the taxes on your behalf. Please contact an accountant for complete details.
  3. Q: What will McDonald’s do with the information that results from the survey?
    A:
    McDonald’s is a customer-driven company. We are always listening to our customers to see how we are doing. We’ll analyze the ongoing results and look for ways that we can improve our commitment to quality, service, cleanliness and value to best suit our customers’ needs.
  4. Q: What will you do with my personal information if I don’t win?
    A:
    The customer’s personal information (Name, address and phone number) will NOT be used for any purpose other than to contact the winners.
  5. Q: What kinds of questions are included in the survey?
    A:
    The questions on the survey cover a number of different areas, but focus primarily on the customers’ opinion as they relate to McDonald’s key drivers of customer satisfaction – quality, service, and cleanliness.
